Can a chandelier fall if spun too many times when cleaning(become unscrewed)?

-Our chandelier fell in our foyer. The contractor claims it was because it was spun too many times while cleaning. Is there a locking mechanism that would prevent this from happening? We rarely touch this fixture so I am shocked that if what he claims is true, that there is not a better safety feature.
